No it isn't a heightmap at all.
There are two possible solutions to create the world.
The first is to create the whole level in an external 3D-Editor like 3DS-Max or Maya or Blender or whatever and to eport the whole level in the game. The advantage is that you don't have to programm a level-editor, becaus you can youse a 3D-Editor. But it's difficult to create new levels. And the user are also not allowed to create their own levels, if they don't have an 3D-Editor and an exporter.
The second - that's what I'm using in my projects - is to create some standard objects(cube, ramp etc.) and then put them all together in a level-file(e.g. a XML-file) and load this file in your game.
