You don't. You use the RAW image that Ogre uses and convert it into a xhf using Flour so it can be read by NxOgre.

You will provide PhysX the data, it doesn't care where it is coming from.

You probably want to read PhysX doc --> Collision detection --> Shapes --> Heightfield Shape.

Have you read the PhysX documentation? I think it is quite clear about how a heightfield shape is created.
